Comfortable oral sex positions: support, communication, and pauses

Neck and torso support, and how to move away, when oral sex is the practice.

Short answer

Comfortable here means the giver can lift their head and the receiver can close their legs or move away. Necks and knees do not have to carry the encounter. There is no oral‑sex position you have to complete.

Raised 69 and sitting on a face do not belong here.

Receiver at the edge, on a seat; giver kneeling

The receiver sits at the edge of a bed or a firm seat, feet on the floor, back supported. The giver kneels in front, with a cushion under their knees.

The person at the edge can stand up and stop. If knees hurt, more cushion or stop. A light chair or one on wheels is not suitable.

Receiver at the edge, face up

The receiver reclines, pelvis near the edge. The giver kneels on the floor, at that height. The receiver’s thighs do not have to hold a head up in the air.

The person with their pelvis at the edge can close their legs or turn and stop. If the edge is high and the giver has to crane their neck, lower the body or change.

Giver face down, pillow under the chest

The giver lies face down, with a firm pillow under chest and shoulders. The weight goes to the mattress, not the neck. The receiver is at mouth height, on a seat or lying on their back.

The giver turns their head and stops. If the pillow pushes the jaw backwards, change it or stop.

Thighs as a headrest

The receiver, on their back. The giver rests their head between the thighs, which act as a cradle, not a press. The giver’s hands stay on the mattress.

The receiver closes slightly and signals, or moves one thigh aside and stops. If you have to tense your neck because thighs do not reach, this is not the arrangement: use the pillow under the chest.

Side oral, giver perpendicular

The receiver lies on their side. The giver is perpendicular, torso resting on the mattress, not in the air. The mouth reaches genitals or anus without either person being behind the other as in spooning.

The giver leans their torso back and stops. If the receiver’s hip blocks access or the neck has to twist, turn a little or change.

Sideways 69

Both people lying on their sides, each head towards the other’s pelvis. The mattress takes the weight. A pillow behind the back or between the knees if needed.

Either can move away by rolling. It does not have to be mutual: the same geometry works for just one person giving oral. If someone has to lift themselves to reach, it is not on your sides.

69 with four points of support, without sitting on the face

The person on top is on four points of support, knees and hands on the mattress, to either side of the head of the person underneath. Weight goes to knees and arms, not the face.

The person underneath turns their head or nudges a hip and stops. Sitting on the face: out. Elevated 69, standing, or held up: out.

Barriers and when to stop

Oral sex can transmit some sexually transmitted infections (STIs). Use a condom or dental dam if that is your choice; have it to hand beforehand. When moving from oral to another practice, use a new barrier.

If your neck, jaw, or knees hurt, stop. Nausea or gag: stop, do not push through. Numbness, dizziness, or shortness of breath: stop.
